Take the Long Island Expressway to exit 51 Head North on Deer Park Ave. Travel heading North for about 2 miles. You will pass the Northern State Parkway. When you come to the fork in the road bare to your right. You now be on East Deer Park Rd. Travel on East Deer Park Rd for about 1 mile. We are in a yellow building on the right hand side next to Ralph's Ices. We are on the 2nd floor. Look for a sign that says Total Home Senior Care
