Take the Long Island Expressway (I-495) to Exit 51 and head north on Deer Park Avenue for about 2 miles. You’ll pass the Northern State Parkway. When you come to the fork in the road, bear right; you’ll now be on East Deer Park Road. Continue on East Deer Park Road for about 1 mile. We’re in the yellow building on the right-hand side, next to Ralph's Ices, on the 2nd floor. Look for the Total Home Senior Care sign.
